Luton & Dunstable Hospital Diabetes Specialist Service

The Diabetes Service at the Luton and Dunstable Hospital provides a comprehensive multi-disciplinary (MDT) service for patients of all age groups with diabetes. The service is provided by seven consultant diabetologists covering medicine for the elderly, paediatrics and adults. The team also includes specialist registrars, a diabetes specialist nursing team, diabetes specialist dietitians and diabetes specialist podiatrists.

The service is based at the Diabetic Centre at the L&D where all MDT services and clinics are held except for Paediatric Diabetes.  In addition to outpatient services, the department also provides a consultation and support services for inpatients.

South Bedfordshire  Integrated Community Diabetes Service (ICDS)

Luton and Dunstable Hospital is the provider for the Bedfordshire ICDS in South Bedfordshire.

Diabetes services independent of Luton and Dunstable Hospital:

Structured education programs for Type 2 diabetes. The new patient education programme 'DESMOND' and 'DESMOND Foundation' is run and managed entirely by NHS Luton and Bedfordshire Services.

NHS Luton has a team of Community DSNs, dietetics and general podiatry services which are directly managed by PCT (providers Cambridge Community Services and SEPT) with plans in place to intergrate further with Specialist Team at the L&D. 

Bedfordshire Diabetic Retinopathy Screening Service is managed and administered by the Bedfordshire Diabetes Retinopathy Screening Service based at Bedford Hospital. The L&D is one of several sites in South Bedfordshire contributing to this service.

Luton and Dunstable Hospital Diabetes Service*

  • Newly diagnosed Type 1 Diabetic or suspected T1
  • Pregnant Type 1 and Type 2 diabetic and gestational diabetics
  • Preconceptional counselling and management
  • Renal patients with complications (renal replacement therapy e.g. dialysis and potential RRT/ transplant and patients referred to renal clinic for assessment)
  • Acute foot problems
  • Management of acutely poorly controlled diabetes in unstable T1 and T2 patients, requiring insulin rescue.
  • Cancer patients (receiving high dose steroids and pancreatic cancer patients)
  • Children and adolescents
  • Acute diabetic emergencies (urgent care pathway)
  • Complex patients with significant co-morbidities (e.g. high dose or long term steroid use, enteral feeding, significant cardiac disease)
  • Active TB patients referred with previously poorly controlled diabetes management.

* Referrals to Luton and Dunstable Hospital Diabetes Service may be triaged to Bedforshire ICDS if appropriate.

Bedfordshire Integrated Community Diabetes Service (Beds ICDS)*

  • Poorly controlled diabetes in otherwise stable patients
  • Chronic: Raised HbA1c not achieving personal target
  • Deteriorating but not requiring urgent intervention
  • Complex conditions/ situations e.g. housebound or living in residential care
  • Complex care, requiring "step down" from acute setting
  • Hard to engage groups and/or patients

*If appropriate, referrals to Beds ICDS may be triaged to be seen in to Luton and Dunstable Hospital Diabetes Service, or referred to L&D for further management after initial assessment by ICDS.

Luton and Dunstable Hospital Diabetes Service

  • Patient not meeting referral criteria for hospital service (see above)
  • Patient who can be managed in Integrated Community Diabetes Service (see criteria above)
  • Patient who can be managed in primary care at practice level

Bedfordshire Integrated Community Diabetes Service (Beds ICDS)

  • Those who require annual review and routine treatments from general practice
  • Those who require inpatient or secondary diabetes care treatment

All urgent/phone referrals should be followed by letter and faxed to Diabetes Reception on 01582 497152 or the consultants' office.
