Care Home Team (BCCG Medicines Management)

The Medicines Management Care Home Team consists of clinical pharmacists and pharmacy technicians who are responsible for, and act as a key contacts for care homes within their localities.  

The technician's role involves supporting and advising the care homes on medicines management issues. This includes:

  • reviewing processes and procedures in the home
  • shadowing medication rounds to support best practice medication administration

The pharmacists are responsible for:

  • conducting annual clinical medication reviews for residents in nursing and residential homes using a multidisciplinary approach
  • providing support with general medicines management issues

The guidance documents below have been developed by the team to support with the delivery of an effective medicines management programme for care homes and older people, and to promote safe and cost-effective use of medicines.
